All persons rotating to or taking an elective at Sinai Hospital MUST complete a registration form and a confidentiality agreement. The registration form, confidentiality agreement and all required documentation must be sent to the Medical Education Office at least two months prior to the requested rotation/elective dates.
Step 1: You need Adobe Acrobat Reader to view and complete the forms. If you do not have Acrobat Reader, click here to download it.
Step 2: All information MUST be completed on-line using Adobe Acrobat Reader; then print and sign the forms. Please do not fill out any information by hand. All information can be entered directly onto the PDF using Acrobat Reader.
Step 3: The forms MUST be faxed, emailed or mailed to the Medical Education Office.
If you are a rotating resident and the Medical Education Office has notified you that you need to be registered as an Unlicenced Medical Practitioner (UMP: training license)
